Feel free to print petition and begin your journey collecting supporter’s signatures.

Once you have decided to collect, please contact CARE and let them know so we can help you along the way and get your information. 

 

• Must be 18 to sign

• Must be resident of Alberta

• All mandatory fields must be filled out

• Cannot sign back of page

• All original copies must be mailed back to CARE, photocopies, emailed, and fax will not be accepted. 

• Please keep in contact with CARE as they will keep you updated with timeline before consultation meeting to insure papers are in on time

• CARE will provide stamps and envelopes to mail back
